Collect feedback about your leadership and service activities

To collect feedback, you should send an email worded similarly to the one below:

Dear [Name],

I am currently completing the Oxley College Leadership Programme and wondered if you would be kind enough to provide feedback about my involvement in [Name of Activity] via the online survey at http://form.jotform.com/form/11311548903 .

The process of seeking and responding to candid feedback will allow me to grow in my capacity as a leader. Your feedback will be confidential with only the teacher in charge of the Leadership Programme able to access the survey results.

Thank you so much for your time.

Once the person you have invited feedback from responds, the feedback will be automatically emailed to Mrs Tregenza who will forward it to you, your Tutor and your Head of House.
